How do I schedule an appointment?
Do you accept insurance?
Yes, we currently accept many insurance plans at this time. We pride ourselves on the amount of time we can dedicate to our patients. If we are not in-network with your insurance, we are happy to assist you in providing an invoice for your to submit to your insurance for reimbursement,
When is payment due?
All self-pay, copays, co-insurance, and deductibles, are expected to be paid at the time of service. Failure to pay, may affect your ability to schedule future appointments. We are a small practice that is dedicated to your health and well-being. Please help us by paying your bills on time.
What forms of payment do you accept?
Visa, Master Card, American Express, Discover, HSA, FSA
Do you charge a no-show or late cancellation fee ?
Yes. We have reserved this time especially for you, please help us by keeping your appointment. We do understand that life happens and plans change, illness occur, please give us at least 48 business hours notice. Appointments cancelled less than 24 (business) hours prior to the time of service, or no-shows, will be charged the full self-pay rate for the appointment that is booked.
